Purpose
Track the lifecycle of every missing or incomplete record.

| Did the record exist? | Identify originating event, series or duty. |
| Who held it? | Record custodian and successor. |
| Was it transferred? | Obtain transfer destination, date and accession/reference. |
| Was it destroyed? | Obtain retention schedule, authority, date and disposal evidence. |
| Was it duplicated? | Search alternative institutional/private holdings. |
No allegation of destruction or concealment should be recorded as established unless the underlying record supports it.
Change history is maintained on the Master Change Log; no separate page-local version footer is maintained.
